Liability Waiver For Participant

As a parent with a child who is a participant in the Lake County Golf Academy Boot Camp instructional programs,  I recognize and acknowledge there are certain risks of physical injury and I agree to assume full risk of any injuries, including death, damages or loss that they may sustain as a result of participating in any and all activities connected with or associated with this program.

I do hereby fully release and discharge Lake County Golf Academy and Bittersweet Golf Course, their officers, agents, servants and employees from any claims from injuries, death, damages or loss which may accrue on account of my child’s participation in this program.
